It can be anywhere in the signal chain. I prefer to wire it after the selector switch and before the volume pot.

Is this unit safe to pair with an onboard bass preamp while using the same battery to power both the preamp and the OD circuit?

using a single battery to power both ---
this may or may not work, depending on whether a 9V battery will supply the current needed for both. 
it is possible that a 9V battery will work for a while, then discharge to the point where it no longer provides the voltage to power one or both devices. 
not a problem if the battery is easily accessible to be changed often. 
i have used a single battery to power both a PT2399 and an LM386 in the same mini homebrew bedroom amp. it works, but it does run down the battery sooner.
if space is a major consideration, and you only have room for one battery to drive both devices, then this may be your only option.

running a preamp into the QDD2 --- 
i don't know about this, because the literature provided in the package does not go into detail about the impedance or strength of the input signal. it is designed to receive a small signal from a guitar pickup, so i can't be sure how it will react to a boosted signal from a preamp.

the only way you're going to be sure is to wire everything up outside the guitar before installing it, and then test it out. 
to do this, you'll need a lot of alligator jumpers. i've done it many times - it's just the fun of experimenting. 
start with your onboard preamp at its lowest level, then gradually try higher settings and see how the QDD2 reacts to the input signal. 
if you reach a point where the preamp causes unwanted distortion in the effect, then your signal is clipping, and you'll have to back off the preamp and leave it there. 
you can also use this test setup to see if the same battery will power both. 
it's a tedious process, but it's always better to know for sure before you start drilling installation holes.

Will this work built into a 2.5w DC 9v mini-amp?

Yes, it will. You will need to wire it before the amp board, which is as easy as soldering the output wires of the QDD2 to the input of the amplifier.

It will need it's own separate battery however, so please keep that in mind.
