Does this have a drain hole on the inside of the tub?

No, there is no drain. If you use water it will develop a ring so you have to clean often. I half filled it with clear acrylic. After it hardened. I placed a miniature duck on top so i could clean the dust off easily.

Would a 12inch doll (barbie) fit nicely in this?

From what I recall, I think it would be a little small for a Barbie doll.

Does the have drain holes or is the bottom water tight?

There are no drain holes, the bottom is solid!
