Does this fit a 4 oz milk carton?

It fits like a capri sun type pouch (drink or like apple sauce). Not square containers.

What size reusable pouch fits?

The measurements are in the product information. But every pouch I have tried fits. The way it’s made, it’s more about the pouch spout locking in place to the top of the holder bc the pouch itself will change size and shape as the child eats but the hard plastic spout on the food pouch does not

Do the Mott's mighty's 3oz fit in this?

I think without lid no juice box can fit in it. If you already buy lid with it then I think yes.
