We actually cut off the collar and disposed of it otherwise you can't move your head down - maybe that's a feature - but it works perfectly well for us without the collar with the chain hanging loose.Since the tightness of the clamps is adjustable, you can get it so it's just tight enough to not fall off but still squeezes the nipples.Very happy with it, I am.
