Q: Will this fit a transponder with an HOV switch?

I don't think they fit 95% of the transponders out there. I wouldn't waste your time.

Q: Which side should the barcode face?

The holder is designed for the flat side of the ez pass to face out. towards the windshield. It works with the barcode to the left or right, rightside up or upside down. Just keep flat side to windshield. Pretty sure it will still work with flatside in. But the holder is not designed that way.

Q: Can I live in another state and only use this periodically in states that have the EZ pass option and while renting a car?

You would have to refer to your state's laws when it comes to that question. This is only the holder that holds an EZ Pass. But the holder does make it easy for you to transfer it from car to car if you wanted to.
