Any single cable coax but of course better quality coax will make for better connections and thus better quality reception.

Yes, it does. If you have an F plug on the end of your cable, it will screw on to the aerial.

It as large adjustable clamp. Ours fits onto a 12 ' pole with ease, on pretty much the tightest fastening.
