Q: Does the headrest fold to be level with the cot or is it always elevated?

It folds to be flat but you will think you’re breaking it-it’s so hard to do. But we lay our flat and it locks into that position.

Q: Can the head rest be tilted up completely? 90 degree angle, I need a shorter length it might work if headrest tilted at 90 degrees!

This cot did NOT tilt 90 degrees.

Q: Does it come with mattress?

The two I purchased did not. However, they have color options and mattress options available. Look at the color choice section. You can also get cot mattresses just about any where on line.

Q: Does anyone know of a camping cot like this for old people who can't get up from 11 inches good?

I would be careful I’ve had it collapse before with people trying to get on or off it. It’s not worth the money in my opinion I ended up throwing mine away in less than a year and we didn’t even use it consistently before the pins started to fall out.
