I would suggest that you know how big your package is, and buy the size that matches. I am built big bought the extra large. The waist has a lot of adjustment , Unless you have a 50 inch waist you will do fine. If you struggle with testicular pain this is a very inexspesive item that worked for me

Does this have the other straps that go between the legs and up the butt?

no it does not. it fits more like a pouch supporter without any leg straps or thong style straps. the waist band fits good and the pouch can be pulled in to give you the right amount of support.

do these lift your junk or just support them
