Do I have to use liquid paint stripper first

You don’t have to use stripper first - if you do you must let the wood dry really thoroughly or the scraper won’t work properly and will snag the grain. If you do use stripper and wire wool you will clear old paint our of the grain and may get a cleaner surface quicker but the grain will be raised - depends what sort of finish you’re trying to achieve. The scraper is an excellent tool to get a perfectly smooth surface.

I need to stripe off old paint from 120years old stair case spindles ?

This scraper is ideal for stripping paint off wood. Not sure how much room you have between the spindles. It could be tricky in tight gaps. Not ideal for curved/shaped areas as well. Other than that a great scraper.

Would this work on painted and waxed MDF?

I have the smaller Bahco 625 scraper and that would definitely remove paint/wax from MDF
