I just purchased and installed this shower head. the led lights stay stuck on red and don't change colors. is this normal?

No, that isn't normal. The one I bought for the kids, changes between red, green and blue, automatically.

Can the LED lights be turned off?

The LED lights can not be turned off when the water is on. The water powers them, so then no water, no lights.

why does the front of it turn yellow after a while? any way to open it and clean it?

I threw it away. Its a cheap gimmick.

can you add this handheld onto an existing wall mounted showerhead?

Yes, I have installed two of these on wall mounted showerheads. I installed them about a year ago and have had no problems with them. If you have kids, you do not want them hanging by the cord, but for normal use, no problem.
