Q: Does it have any structure in it? Or is it just a bag? Im wondering how well it holds up when you lean back on it.

Chair does have structure, not a bag. It holds up when you lean back on it. We bought it for one of my daughter's friends (8th grader) and she loves it. She does homework in it, listens to music in it, just hangs out in it. I checked with her today and asked her about your questions to make sure I gave you an accurate representation of the chair.

Q: What are the "beans" inside made of? My daughter has a severe nut allergy and I've heard that some bean bags are filled with nut shells.

I think those beans are actually made of small pieces of Cork :)

Q: can it be used for a medium size dog?

The dimensions are 33.0 x 32.0 x 25.0" they can fit most large adults. In my opinion, even if your dog is a Great Dane, it should be ok. Hope this helps.

Q: can this be used on an outdoor patio?

If all you want is a lumpy floor mat, yes. If you want something that functions as a chair, buy something else.
