Would this fit in an arep spacer woth mask and blue inher?

Yes, I fit in it my childs spacer and blue inhaler. Am really happy with it, it's a great medical bag and keeps everything all in one place.

Could I store 2 standard sized EpiPens in this?

I don't know how big an epipen is, but I keep a 6" long aero chamber (like the one in the photo), plus other medication, in mine and it fits perfectly. Hope this helps.

How should the inside and outside of this item be cleaned before first use and going forward after that as well?

As the product is made of a waterproof style fabric I clean mine by spraying with antibacterial spray and wiping over with a damp cloth and just leave to air dry. Hope that helps

Is this big enough for an aero flow spacer and two inhalers?

Yes, the medical bag will take a 6" aero chamber and 2 (possibly 3) inhalers.
