Does the guitar need to be set up or is it ready to go?

The Baby Taylor needs no set up, just tune it and it's ready to go. However you may want to adjust the action height of the strings if it feels hard on your finger tips while fretting. You can learn how it's done on You Tube and do it yourself.

Can this be used with a strap?

Yes of course, I use strap for more confortable guitar playing...

Wife is 5 foot 1 inch small hands She is using a full size and is uncomfortable to play. Would a 3/4 be better

3/4 size guitars are a great option for people with smaller hands, and are much more comfortable to play while sitting down. The Taylor BT2 is a very comfortable, easy to play option for people of smaller stature!
