I have Briggs and Stratton 3.5hp on a rototiller the recoil rope doesn't "catch" is this what I need?

If what you mean by "doesn't catch" is that the rope doesn't rewind when you pull it to start, then yes. That's the exact problem I had, and replacing this part took care of the problem.

I have a 5 hp Briggs that is around 25 years old. This looks like the same one. Will it work on it. It is on a rototiller

yes it should I installed one of these on a 5 horsepower Briggs and Stratton it works great

Does this fit a riding mower engine 19.5?

Riding mowers usually have a starter, so I'm not sure

does the ball bearings come inside this???

Yes is complete, threads on may use flywheel holder and tool to tighten
