Q: Can you drink without topper?

Yes, these can be used without the toppers, but the toppers will prevent spills.

Q: Is the top spout like a straw or a squirt top? How do the kids drink it from the topper?

The tops do not come with these. I buy these individually or in smaller packs from target, Walmart, or grocery stores and then wash and reuse the tops with these refills. The tops are one way straws that somehow do not spill.

These refills do not come with the toppers.

Q: Are the bottles reusable? And dishwasher safe?

I've never tried to wash the bottles, but I do put the character tops in the dishwasher (top-rack). No issues so far. But I can't speak for the company, as to whether they are "officially" dishwasher safe.
