Q: Is the head on the 3/16", the same size as the head on the 1/4"?

no there is a slight size differance......... like no more then 1/8" the 1/4 " size has a prety big head and a length of over 4 " sometimes that might be a little too much flr a 2 x 4 wall space of only 3 1/2 "

Q: Can it be confirmed these are actual stainless steel as advertised?

Its strong; good to hang a heavy flat screen on the wall

Q: Is there anyway to get these out of the wall after installation?

Not really. I was once able to retrieve the winged nut of the anchor with a magnet ... but it took about an hour to wiggle it around to the hole so I could retrieve with needle nose pliers. Typically, I just use another winged nut and reinstall if I have to the bolt from the wall.

Q: How much weightis is it rated for ?

for the 1/4" by 3", the box suggests up to 120 lbs each, but ultimately it you'll want to take your substrate and application into consideration. The 120 lbs assumes a downward force on a stable vertical substrate. Hanging from the ceiling you will want to use considerably less weight. Similarly if you have a really thin substrate.
