Q: Do you determine size by measuring front or back paws? Back ones are much longer.

I'd use the larger paw measurement. I purchased the medium size for my 32 lb elderly sheltie and they fit perfectly. He doesn't slip on hardwood floors now and moves more now. I thought he'd pull the socks off but he leaves them alone. Great purchase!

Yes, I would think that they would be helpful for her. They have been helpful for our (elderly) dog (after she had a stroke, she has been having issues with her equilibrium and gait) - she doesn't slip and seems to get around much better once they are on . The only trick is actually getting them on - they are supposed to be snug so that they don't fall off while she is walking around, but it seems to consistently be a 2 person job to get them on (one person to hold her foot steady and one person to actually get the sock on). She isn't a big fan of that whole process and she walks funny for the first minute or 2 after they are on but then she remembers how much easier they make it for her to walk around, and then she has no problem and things are golden :)

Q: What size should I get for a 123lb Great Dane?

There was a size chart with what measurements to use. Hopefully they still have that, if your dog is in between sizes I would go up.
