Q: Does it have a perfectly smooth flat bottom?

yes it heats very fast would order again bottom is flat cleans easy

Q: Is it normal for the coating underneath the pan to show white streaks for where it sat on the burner plates? Flame is never above medium.

I have had mine for about 2 months and I use it on an an electric coil type of burner. There have not been any such streaks on mine yet, no marks at all, and I use it about every other day.

Diameter is ~8.5 inches, height is ~4 inches without lid, 6.5 inches with lid
