Q: Will this work with Verizon S5 SM-G900V?

It worked perfectly for my Tmobile S5 so it should work for the Verizon version. I don't see the manufacturer changing out the connector pins on differing carrier versions.

Q: How is the receiver installed? It did not come with instructions.

You just line it up with the 3 metal prongs inside your phone. The prongs should be located on the left hand side of your phone, next to the battery, when looking at it from the rear with the back casing off of your phone. Maybe use a little piece of tape to keep it in place and then put the cover back on your phone. Should be good to go after that.

Q: Does this change the thickness of the phone? Will it affect the fit of protective cases?

Yes, it adds thickness to the phone which pushes the back cover away by the heart rate monitor sensor which I believe makes it vulnerable to liquid.
