They fit all standard size Plantronics headsets, and the ones you are mentioning are.

Q: does this fit Plantronics s12?

I believe so, that headset is similair to the one I used them on. The cushions were a little snug but worked better then the foam covers I replaced.

Q: would they fit a 2004 nissan quest headset?

I just measured the ones I installed and it appears to be 2" (inch) round. There is some room to play so a 1 3/4" to maybe 2 1/4" head piece would be okay. All headsets are pretty much built the same so you don't have to worry about the way they fit. When you take an old one off, if still attached, or look at the new one, it should be obvious how to put the new cushions on. I use mine all day Monday through Friday and they are far and away better in comfort and durability than any I have ever tried before. If the unit is about 2", you can't go wrong with these. I’m not familiar with that specific headset. You should measure your headset diameter. These specific cushions are 50mm diameter and have a mounting diameter of 39mm. If your headset is bigger then you should purchase the next size. 60mm
