Q: How many charges does it give the iPhone 11?

It would be one full charge not 1.5. iPhone 11 is 3110mah battery. Powerbanks typically transfer 60 to 70% power so the entire 4500 will not transfer. Expect a full charge and maybe a little more. But not 1.5 charges.

Q: Are the lights which indicate how much battery the charger has supposed to be on the whole time or is my charger defective?

They stay on while charging a phone or while being charged itself. They stay on for a while when not in use but should then go off after not been touched for a while.

Q: Is there any chance this could cause the phone port to become loose? The phone can't lay flat or the charger weighs the port down when holding it

You do have to be careful inserting the charger into the phone socket. I haven't had any problem with the phone port
