Not likely unless your uke can also directly drive headphones. Unless your ukulele has an internal amplifier (not a preamplifier) capable of driving headphones, then the iPhone level will be far too powerful compared to the instrument's output for them to ever achieve anything close to a "mix" with a passive device like this mixer. An active mixer will do exactly what you're trying to do, but make sure any such mixer is equipped with a preamp suitable for your uke's pickup or you may not be able to get enough volume from the instrument in your mix.

Q: Is there a headphone monitoring port?

No, there is no headphone output, but there are two line outputs (1/4" and 1/8") and one of them could feed a headphone amplifier.
