With a CATS, you have to pull it tight and then twist. This one locks in place, so you can self apply without having to pull tight and hold or velcro. It's a minor difference, but enough of a change that I've updated my primary first aid kits to include this tourniquet. The CATS are still a great tool, but I will keep those in my secondary kits.
