Q: Im sure I’m just misunderstanding but aren’t these labeled upside down? Shouldn’t the smallest number dose be at the bottom?

The dispenser-end is pictured at the bottom; and, there are threads at that opening that fit over the "spout" of the cream and twist to "lock" the spout in place. Then the cream is drawn into the applicator by slightly squeezing the bottom of the tube of cream while simultaneously drawing the plunger of the applicator. Regardless of whether you put the tube of cream at the top and draw the cream down, or put the applicator at the top and draw the cream up, AS YOU DRAW THE PLUNGER, and the cream enters the applicator, it is the dosage-line printed on the plunger that shows the amount drawn from least to greatest. (My doctor didn't give me a particular dosage-in-grams. Instead, she just said to "use a pea-sized amount", so I just eyeball it as it comes out of the tube and into the applicator.) Me: "Is that about the size of a pea? I guess so!"]

Q: What is the circumference of the opening?

I don't know, but it fits estrodial cream tube and monistat cream tube.

Q: Are the edges rounded on these?

For me, they seemed more rounded than the one you get with the cream. However, they are not completely round so you may still feel a little scrap. Also, they are longer than the ones you get with the cream, so be prepared for that.

Q: How many come in one order?
