Just put the big loop through the small loop and pull tight.

Q: As the Plus+ can track sleep, does this mean it can stay connected catching mons and spinning Stops/Gyms for more than an hour without disconnecting?

No. Actually when you put it into the sleep tracker mode it disconnects and does not catch Pokemon. Sleep tracker mode is "offline", then it syncs to the game once you wake up and connect it again
