Q: Is it just a single piece of flat rubber or is it three dimensional?

It's 3 dimensional , it was a stocking filler for my husband unfortuantly the loop to hold it on to the actual key chain is rubber and it snapped off really easy , wish I had just went with the metal NES one I was originally going to go with. If you use your keys several times a day I wouldn't recommend hope this helps.

Q: Is the image on both sides?

It's not an image, theres raised rubber for the buttons and directional pad, and no it's not on both sides.

Q: are the buttons pressable?

No, the buttons do not have any function or noise
