Q: Will this step the USB down from 240v to 5v? I know it won't for the 110v receptacles.

Hi this provides portable surge protection on 110v lines. It does not step down AC voltage. It cannot be used on 220-240v Without a seperate step down transformer. Usb output is in DC voltage and is comparable to charging your phone via usb cable to your computer. The amperage on the usb outlets are high enough to charge tablets in good time.

Q: Can you use it a two prong plug, by leaving the middle prong folded down?

I just checked. It folds down flat for travel, but when you lift up the two prongs, the middle one pops up a little bit so that the two prongs can't plug in. The only way would be to break off the third (ground) prong, which would reduce its ability as a surge protector.
