They cannot be felt through the mattress. I worried about the same thing, but they work great and we have not heard of any complaints from our Dad. I like them because there was no danger of the rails coming out from under the mattress when he leans or hoists himself up with them.

Q: can you put this on a adjustable bed that does not have a box spring?

Should work fine . I bought this for my sister’s Adjustable bed. She does have a box spring but should be fine without. She loves it for getting out of bed and it preventing her from falling out!
