Q: The panel has a mic blend knob, but there is no other mention of it in the description. If there is a mic input, what type is it?

The mic is under the saddle. The blend knob changes the level from the bridge and the acoustics. It is a ver effective control.

Q: Do you need to cut/sand the saddle down to keep the strings at the same action level, since you are adding the piezo under the saddle... ?

Yes absolutely. Lay a sheet of sandpaper on a flat surface. Lay the saddle bottom on the sandpaper and strike the saddle across the sandpaper keeping it very flat. Do not rock the edges or the ends rounding them over. Keep it flat. Remember which end is the treble and which end is the bass, they are not interchangeable. The bass strings side is supposed to be taller. Mark the side of the saddle with a black sharpie. Strike a sharp reference line with a razor knife in the black marked area of the saddle side as reference mark to your progress. Use a feeler guage the approximate thickness of the piazo pickup, slightly less allowing for the thickness od the razor blade. Remove the bolt from the feeler guage set and lay the single guage flat on the surface. Lay the saddle flat on the surface next to it . Strike a sharp line with the razor knife on the marked side of the saddle across the top of the thickness guage. Be careful of cutting your fingers here. Draw the saddle back and forth on the sandpaper keeping it absolutely flat. As you get close the the mark test the saddle in the guitar 🎸 multiple times. Creep up on your final height adjustment. You can sand more later but if you go too low now your strings will buzz and you will have to get a new saddle and start over. Use a capo on the 1st or second fret to hold the strings in place on the peghead so you don't lose them as you loosen the strings enough to pull out the ball end from the saddle. Easy peasy to test and redo this way.

Q: Does the output jack go through the endpin?

Yes you can put it in the end pin I have also put in bottom of the side with a support block

Q: Does this come with the piezo pickup and output jack, or is it just the preamp?

Yes it has both, once you receive it you will see that both are pictured. The output jack is also the on/off switch , guitar cable in unit is on, cable out unit is off.
