Q: Can you use this without the middle shelf?

Yes. If you only wanted two shelves you would not use 4 poles and the middle shelf. However, the cart would be shorter and the spacing between the shelves would be the same as if you used all three shelves.

Q: Is it possible to use/assemble without wheels?

Yes if you prefer, the cart can stand without wheels. What's more the cart can be stopped in one place with the help of 2 lockable wheels.

Q: Can the top shelf be inserted upside down so I have a flat area to place my laptop?

I believe it could be done, but I don't know how stable the cart would be. Especially since it has wheels, you might have trouble keeping it in place while you type on your laptop. The laptop would need to be a small one, also. The cart is wonderful, and you had a good idea, but I'm not so sure it would work well with this one.
