Q: What size is the gel pack that comes with this? I want to order a 2nd gel pack. Thanks.

The gel pack is 10" x approx. 4.25".

Q: I want to use this on my lower back. Will it fit around a 42" hip area?

I believe it will work. Might be a little snug.

Q: how is the gel pack held in the belt so it does't fall out?

There is a packet inside the belt to slide in the gel pack and with the belt tight to the body it can't fall out.

Q: is the pouch large enough to use a blue ice pack like the Hard thick kind that go in coolers ? dimensions 6.5 "long 3.5" wide

The actual pouch measures about 10" x 6". It's made of a stretchy mesh. I have a large ice pack in mine, but the mesh isn't strong enough to hold it in place, so I wear the belt tight enough to keep it secure.
