Q: how long does the cap maintain a therapeutic cold temperature?

Um, no it does NOT stay cold for 2 hours Stacy who writes lots of reviews on the same days. It stays cold for MAYBE 15 - 20 minutes. While it is cold it feels great but don't expect it to last longer than that. I hate lies.

Q: Why does the bag ( that I tore open) say that it must be stored in the bag in the freezer? To prevent what type of damage?

I think it says somewhere to avoid the Cranial Cap from getting water on it so perhaps that's why you're supposed to keep it in the plastic bag? Perhaps you want to contact the company.

Q: Does it work well for holding other ice packs in place

Never tried that and it's not designed to do that. Since it is itself a cold pack, not sure why you would want to place other ice packs in with it. I suppose if you are lying down, holding still, it might work.
