Q: Does it sag? I want to put in inside my apartment for the winter. And how many items can it hold without hangers?

Yes it will sag. No way it will support a full washer load. We use it spanning our utility closet (6') for my wife's lingerie that can't go in the dryer. Get a ventless Miele dryer, which uses 110v.

Q: Can it be used outdoors and can it be mounted on a 4x4 post?

We found must be mounted as shown which would make it too wide for a post. We tried to mount it sideways and also upside down and outer cover piece kept falling off the mounted base. Not sure about outside as we put in laundry room inside house. But really like it as nice and sturdy. Holds a lot of clothes.

Q: Is the length the clothes line extends adjustable if the width between walls isn't the exact distance in width as the clothesline?

Yes. The clothesline will lock at any distance less than it's length.
