Q: Is this emergency ladder a 1 time use? I read someones review that said that but I've never heard of that before.

This question/answer is a little bizarre. Would one-time use be too little to justify its purchase if it saved your life? As to running drills, please be clear if this involves actually lowering and descending the ladder? If so, this is fairly stupid and surely not what is intended. A drill means getting the ladder from it's storage place, opening the window in the dark and attaching it to the window sill. That's all. The likelihood of injury any time one uses something like this is high. The point is to avoid dying. Falling a dozen feet won't kill most people--especially if they are upright as one would be going out a window on this thing. It is valuable because it helps you get out the window. If you fall off it once outside and break a leg, you are lucky and the ladder did its job. This is a FIRE situation, a catastrophe. Let's be realistic. I would recommend attaching a pair of work gloves to the stored ladder to help in holding on to a chain.

Q: do you need a window sill or some kind of lip on the window in order for it to attach securely ?

It appears you at least need a window that is a minimum of 8-12" off the floor which you intend to use this ladder. The stair goes "over" the lip of the window (whether it has a thick sill or not is kind of irrelevant), but unless the window is at floor level this should work fine. I tried it on multiple windows (high and low) on 3 different stories of a house with varying types of windows (casement, double hung, etc..) and it worked fine. I also tried it just hung over the rail of a deck and it held fine...so I'm guessing it would work on most windows.

The single ladder is heavily discounted.

Q: Our window sill is 23" deep. Can this ladder attach to the sill?

No. The bend/hook that supports the ladder steps will not fit a 23 inch wide window.
