The widest part of the loop. ( ring) is 12mm wide.

Q: Will this fit iphone11 pro? Does it a screen cover?

as long as your case/cover has a hole to feed the tag through it will fit. the strength/stability of the lanyard is dependent on the case/cover. f you don't use one this is not for you.

Q: I have read you need a hard case. Is it ok with a hard case that has a soft tpu edge, like the Spigen?

I have a spigen phone case and it works fine on mine, some cases might be different, mines hardwaring, I've used this lanyard for two years now & not had a problem with it slipping out. You coukd akways but it, try & send it back if it doesn't fit or work, I gave mine a few medium tugs before I went around my day to day routine in work. X
