It does really stick out. It's like a rubbery bib so you'll need a shirt over it.

Q: Does flesh colored cover black people too?

I looked around for different skin tone colors. The shirt I used, I made the hole big enough so you could just see the alien, put alot of fake blood around so the skin tone didn’t matter.

Q: Is this three dimensional?

Yes, the head is plastic (just like the rest of it) and it protrudes about three inches.
