No. It does not have metal. I found this pillow very helpful.

Q: Will this work for a client who has issues laying face down with normal massage pillow?

I can't say for sure; it would probably depend on the individual client and his/her specific issues or concerns. However, you lie on it face-down just like a normal massage pillow or a table at the physical therapist's office that has a face-hole, so I'm not really sure there's a huge difference between to the two. Your client might very well have the same problem with this. Please note, I'm not a massage therapist and have no expertise regarding tools and such that massage therapists use. My physical therapist's office had this exact pillow. If there weren't enough tables with a face-hole, they had this that you could put on a regular table and lay face-down to do any exercises. I liked it so much that I bought one for myself to continue doing my exercises with and to lie face-down on for 20 minutes at a time during neck spasms or while using my TENS unit.

Q: Can the face pillow be removed to apply a cover?

Both cushions attach with velcro. I don't see why you couldn't apply a cover, although I have no experience using covers.. I've seen some covers for prone pillows/face cushions that you could use a cover on even if the pillow itself was not removable. As a former physical therapy patient, I use mine for personal use, although in physical therapy, we would wipe down the prone pillow (and tables, equipment, etc.) with disinfectant after each use.
