Q: Two questions. One , do you have to assemble it yourself ? And Two, if you don’t have to assemble , does it come with the little thingy on the back ?

No assembly required, and yes, it comes with the hanger on the back so you can hang it on the wall.

Q: what’s the included metal hook(?) for? is it to hang the shelf?

You can hang it. I just sat mine on cabinet.

Q: Can you remove the shelves?

No...they are permanently in place

Q: Would I be able to fit eyeshadow palettes on it?

I don't think so, the shelves are small and fit a chapstick, some hair clips and a bottle of perfume. So it's more decorative than good for storage of things
