Q: How far apart are the screw holes on this?

There are two sets of screws 3 3/4" and 91/2" center to center

Q: DOES THIS REFILL CONTAIN THE SCREWS AS WELL?

The mop refill has two plastic bolts that are part of the molded base that holds the sponge. Also included are two plastic nuts to tighten the refill. If you find the holes for the bolts are too close, simply drill two new holes in the metal base so the refill mop head will fit!

Q: Where on the mop can I find the model number?

First you disassemble the mop from the mop handle by turning the two wing nuts 90 degrees counter clockwise. When the mop (sponge) come out the number will be on the outboard side of one of the wing nuts.

Q: Does this have a plastic base on it where you attach the sponge to the mop?

Yes, and it comes with plastic washers to attach the sponge to the mop, too.
